Requested Materials - This information is REQUIRED and may include voter certificates on vote-by-mail envelopes, cure affidavits, corresponding comparison signatures, duplicate ballots, and corresponding originals.

Viewing Schedule

Requests must be made at least 48 hours in advance of the requested time. Your request is not confirmed until you have received confirmation from our office.

This form becomes a public record when submitted to the Supervisor of Elections.
